Tell them what it was really like.

Seven short questions about a service you, or someone you care for, used. It’s anonymous. We group the answers and share them with the people who run those services.

A listening circle in a church hall. Mostly Black people of different ages sit together while an older Black woman speaks.

How your answer stays anonymous

  • No name, no number

This form never asks who you are, and we don’t store anything that links back to you.

  • Names hidden first

If you mention names, numbers or addresses, we hide them before anyone on our team reads your answer.

  • Kept for two years

Then your answer is deleted. Only the anonymous totals stay.

1. Which service is this about?
2. When was it?
3. Did you feel listened to?
4. Were you treated fairly, whatever your background?
5. Were you given information in a language you understood?

Please don’t include names, phone numbers or addresses.

7. About you (optional)

This helps services see who they are and aren’t reaching. Skip anything you like.

This web form is the most private way to take part. If you answer on WhatsApp instead, Meta can see that your number messaged us, though our staff can’t link your answers to it.
